Please describe the matter as fully as possible, covering who, what, when, where, why and how.

- Give the real name of the person concerned wherever possible

- Describe in detail when and where the problem occurred

- Supporting material such as photos or documents is needed. If the wrongdoing is clear but you have no evidence, describe how you learned of it and where and how the facts can be confirmed
